For each funding source, identify the following:

1. Eligibility criteria for the program - including individual disability-related criteria, income and resource rules, and age guidelines;
2. Eligibility criteria for a specific assistive technology device - including covered services, application process, approved vendors or providers; and
3. Availability of an appeal - including the process and any deadline for requesting an appeal.
